If you weren't sure how this works, or just want to try something else, here's a typical hammer build which works around provoke and charged up.

It works really nicely, especially for mowing down a lot of packs, but bear in mind you'll be using smaller attacks without a cooldown (so no need to stack CDR), but thankfully that means shorter cooldowns too!
